For Package's Freebies ordered for pick-up requesting for delivery:

Date Availed:
Recipient Name:
Recipient Complete Address:
Contact Number/s:
*Shipping fee will be quoted once the details were provided
* A Letter of Authorization is needed if these freebies will be delivered to a different name. Attach a picture of a government ID with 3 specimen signatures affixed.
