- Letter of request (indicate that you are settling down here in the Philippines)
- Proof of Billing stating the Address of your current residence.
- Latest Brgy. Clearance
- Picture of a Government issued ID with 3 specimen signatures
- Certificate of Employment (any certificate that you have worked abroad)
